Lake Huron Medical Center PHO Patient Centered Medical Home

To ensure that Lake Huron Medical Center Physician Hospital Organization (LHMC PHO) is providing the highest quality in treating patients, LHMC PHO has adopted a plan developed in 1967 by the Academy of Pediatrics. This plan is fully supported by Blue Cross Blue Shield of Michigan (BCBSM), and is known as the Patient Centered Medical Home (PCMH). With much of health care spending coming from hospital visitations (both emergency room and in-patient) and lack of coordination of specialty care versus visits to a primary care physician for treatment, the PCMH will establish an interactive patient to physician relationship. The intent of this relationship is to create a dual responsibility (patient and physician) for a commitment to health at a lower cost.

Our community of physicians has a multi-specialty care team with a shared obligation towards managing their child, youth, adult, and senior patient members. To further the success of the PCMH, patient data provided by a patient registry will allow the care team to access clinical data efficiently and give the physicians the ability to analyze their own performance. Physicians can use the registry to coordinate with their patients regarding health status and responsibility. This active collaboration between the care team and patients can aide in the management of the patient’s health, while promoting the development healthy behaviors.

The key to the success of the PCMH is communication. The transmission of accurate and essential health information between patient and physician also extends to the physician care team and clinicians. By committing to co-management of health, as presented by the PCMH model, LHMC PHO strives toward an effective treatment of its patient community – providing access to optimal primary care while increasing overall health status and ensuring cost savings.

Lake Huron Medical Center PHO is committed to assisting their Primary Care Physicians in achieving designation as a Patient Centered Medical Home. By participating in LHMC PHO’s in-office workshops and monthly Learning Collaborative meetings – practices can achieve PCMH status.
